TL;DR

The Clinic is where HRT happens. You need a referral first (it arrives by SMS once you start taking hormones at home or hit the Fem 25 milestone), then you register, then you cycle through five Dr. Mercer consultations as doctor_stage advances. Every three days you book a treatment-room slot for an estradiol injection. The pharmacy downstairs sells refills in boxes of 3 doses. The prostate room is an optional cash-flow side door that drains Arousal completely.

Note: laser hair removal happens at the Beauty Salon, not here. Cage downsizing and chastity unlocks aren’t clinic procedures either, they’re Master-route bar orders and self-discovery beats.

The HRT pipeline

Five clinic stages run in order. Each consult costs $30 (first) or $20 (later), takes 10 Energy, and gates the next.

Stage 5 is the steady-state loop. The “Personal” variant locks the clinic door for an after-hours session. The “BodyScan” variant gets clinical about the cage if you’re wearing one.

Treatment room (the injection itself).

Costs $10, 8 Energy. Requires hrt_prescription && hrt_doses_remaining > 0. Locked for 3 days after each shot. Each injection ticks hrt_injections += 1, drains a dose, gives +2 Fem and +3 Arousal, and resets the missed-dose counter.

hrt_stage (the body-change progression, separate from doctor_stage) auto-advances on injection: - Stage 1: first injection - Stage 2: 5 injections AND Fem >= 30 - Stage 3: 10 injections AND Fem >= 50 - Stage 4: 20 injections AND Fem >= 70

First injection plays its own scene (ClinicTreatment_First). After that it’s the routine version with stage-aware flavor.

Pharmacy (prescription and refills).

First pickup is free, sets hrt_prescription = true, adds 3 doses. Subsequent refills cost $25 for 3 more doses. Hub gates the refill button when you’ve still got doses left, so you can’t stockpile.

If you were taking oral hormones at home, the pharmacist tells you to drop the pills (the injectable form is strictly stronger).

Lab (blood work).

$15 before you’re prescribed, free after. 5 Energy. Dr. Mercer reviews the panel offline. No stat gains, just a way to burn time productively if you’re killing minutes before another appointment.

Prostate fluid donation

Side gig. The receptionist runs a “university study” out of a private room with a padded table and a collection cup. Pays $25 per session. Requires Arousal >= 50 to enter. 10 Energy, +3 Sub, +2 Fem. Hard-resets Arousal to 0 and clears the visibly-aroused flag, so this doubles as a release valve when you’re stuck at high Arousal with nowhere good to drain it.

Clinical (ClinicProstate_Clinical). The default. Professional, efficient, you leave shaking and empty.
Thorough (ClinicProstate_Thorough). #shot:hero. Hormone-aware: stage 3+ adds a sensitivity beat. The orgasm “detonates.”
Verbal (ClinicProstate_Verbal). The nurse is conversational. +1 Shame.
Chastity (ClinicProstate_Chastity). Branches hard on wearing_chastity. With cage: hands-free prostate-only orgasm, +1 Sub, +2 Shame. Without cage: the nurse pitches you on getting one.

Dr. Mercer

The clinic’s resident. Auburn hair, wire-rimmed glasses, mid-thirties. Three portrait variants: dr_mercer (neutral), dr_mercer_warm (most scenes), dr_mercer_proud (stage 4+). She starts every interaction professional and ends most of them with her professionalism slipping just slightly. The boundary is intact through stage 3. By the stage 5 “Personal” variant she’s locking the door after hours.

She doesn’t comment on the cage in clinical contexts beyond noting it’s “ideal for the procedure.” If you visit her stage 4+ wearing one, the exam gets longer.

Waiting-room shuffle

40% chance per Reception visit, 4-event shuffle through clinic_waiting_event_cycle:

Magazine: transitioning before/after photos, +1 Fem.
Stranger: a confident trans woman tells you Dr. Mercer is amazing, +1 Fem.
Nervous: bouncing leg, a calm bystander nods at you. No stat change.
Mirror: glass-partition reflection. Stage 2+ adds a “the hormones are working” beat.

Tips and gotchas

You need the referral first

Visiting before clinic_referral = true lands you at ClinicEntry_NoReferral (locked-out stub). The referral fires automatically once you’ve been taking oral hormones for a few days (taking_hormones + Day 14+ gate) OR you hit the Fem 25 milestone. Watch your SMS.

Miss an injection and the doctor will notice

The 3-day cooldown is a floor, not a ceiling. Skip too many and hrt_missed_count climbs. SMS reminders fire from MorningEvents.ink once you’re 4+ days late. The treatment room itself locks until you come back.

Prostate sessions are the best Arousal vent in the game

Spa cold plunge is -25 and unlimited but only fires at Arousal >= 40. Home cold shower is -15, once a day. Prostate donation zeroes Arousal AND pays $25 AND requires Arousal >= 50 to enter. If you’re sitting at 60+ Arousal with nowhere good to go, the clinic is the answer.

The prostate room narrates the cage you're wearing

ClinicProstate_Chastity is the only variant that meaningfully branches on wearing_chastity. The other three play their text either way. If the cage is part of your run identity and you want it acknowledged, that’s the variant to wait for.
